mysql not authenticating

Just curious if anyone's seen this before...
MySQL was up and running apparently just fine, however the app would not 
authenticate users. I restarted mysql from its initscript and still not 
change. In the end I restored the mysql.user table and it once again 
started authenticating scripts trying to access the various databases.

Anyone seen this before?
