Do you possibly have to enable the other 2 buttons on your 5-button wheel mouse? Like, in XF86Config's pointer section add a "Buttons 5" and maybe even assign button4 and button5 to do something? I seem to remember doing that once, forgetting how, and never looking it up again... :)
