Apache SSL not working - what's missing?

I have gotten SSL to work in the past with no problem.  But
it seems like there was more to it than the current notes I
am working off of.

Could someone look at this link and tell me what they've
left out?  I've done a "rm -rf apache" and "make
certificate && make install" about 6 times on two different
machines and I keep getting verious errors.
